PAS 300

A. DEFINITION OF ACCIDENT

  • Loss as a direct result, independently and exclusively of all other causes, of bodily injury effected solely and directly by an external, violent, and purely accidental means and evidenced by a visible bruise or wound on the exterior of the body except in the case of drowning or if internal injury revealed by autopsy.

  • Accident or accidental referred hereto shall mean sudden, unforeseen and fortuitous event;

    • 2.1 The death or bodily injury occurred within 180 days from the date of sustaining such injury or accident;

    • 2.2 The injury was sustained while this Policy is in effect.

B. BENEFITS (per card)

  • DEATH OR DISMEMBERMET

    In case of Death or Dismemberment due to an accident, the beneficiary or insured shall receive the amount specified below:

    Loss of life 50,000
    Loss of entire sight of both eyes 50,000
    Loss of both hands or both feet 50,000
    Loss of one hand and one foot 50,000
    Loss of either hand or foot & sight of one eye 50,000
    Loss of arm at or above elbow 35,000
    Loss of leg at or above knee 30,000
    Loss of one hand at or above wrist 25,000
    Loss of one foot at or above the ankle 25,000
    Loss of hearing of both ears 25,000
    Loss of sight of one eye 25,000
    Loss of four fingers and thumb of one hand 20,000
    Loss of four fingers 17,500
    Loss of hearing of one ear 12,500
    Loss of all toes on one foot 12,500
    Loss of thumb 7,500
    Loss of index finger 5,000
    Loss of middle finger 3,000
    Loss of ring finger 2,500
    Loss of big toe 2,500
    Loss of little finger 2,000
    Loss of any toe, other than big toe, each 500
  • MEDICAL REIMBURSEMENT

    In case of an accident, the insured can reimburse the medical expenses incurred up to P5,000.

  • DAILY HOSPITAL INCOME

    In case of confinement in a hospital due to an accident, the insured will be paid P500 per day up to a maximum of 60 days.

  • BURIAL BENEFIT

    If the insured dies due to an accident, the beneficiary shall be paid an additional P5,000 burial benefit, on top of the P50,000 Accidental Death benefit.

C. ELIGIBILITY REQUIREMENTS

  • Qualified Ages - eighteen (18) to sixty (60) years old

  • Must not be engaged in any of the following occupations:

    Military/police men, paramilitary/secret service personnel, detectives, firemen, explosive makers, sailors, divers, fishermen, off-shore/gas rig workers, loggers, miners, underground workers, construction workers, window cleaners, cable men or linemen

D. ADMINISTRATION AND CONSIDERATIONS

  • The PAS300 kit may be purchased at any CHSI branch for a one-time payment of Php300.

  • The CLIC Application for Personal Accident Insurance which is attached in the Personal Accident kit must be duly accomplished and submitted to Caritas Life directly or through any CHSI Branch office within six (6) months of purchase date.

  • Effective Date of Insurance coverage shall be one (1) day after receipt of application or one (1) day after card has been paid, whichever is later, and automatically expires one (1) year from effective date.

  • A Confirmation of Coverage (CoC) shall be issued upon receipt of application by Caritas Life.
